An exciting opportunity has arisen for a Band 5 OT to join the Birmingham Community Healthcare Foundation Trust Therapy Hub. The Therapy Hub is a community based team comprising occupational therapists, physiotherapists, rehabilitation technicians, rehabilitation assistants and administrative support.
As part of the Therapy Hub, you will be responsible for maximising the independence and autonomy of people, ensuring that interventions are provided fairly and in a manner that is sensitive to individual needs. You will have good assessment and treatment planning skills, an ability to prioritise work load and enjoy a multi-disciplinary rehabilitation approach. You will work in partnership with the Early Intervention Community Team to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.
Main duties of the job
The post holder will ensure access to urgent and routine Occupational Therapy assessment and intervention provide and monitor treatment programmes at home and ensure safe discharge or referral on to alternative services as appropriate. They will work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need.
This is a community post so access to reliable transport or means of travel is essential.
The role provides an exciting challenge, supported by a comprehensive supervision, training and development programme including a Preceptorship programme for newly qualified staff and mentorship for our new recruits.
The post holder will be expected to work multi professionally, support other team members' roles and contribute to the future development of the service providing professional advice and appropriate training to others in the multi-disciplinary service.
