The Advanced Clinical Practitioner will be a highly competent, visible autonomous practitioner,capable of functioning effectively across the four pillars of advanced practice as outlined in theMulti-professional framework for Advanced Practice in England (2017); clinical practice;leadership and management; education and training; research, quality improvement andservice development.Along with the Divisional Medical Director, and Professional and service line seniorleadership, they will provide positive leadership, connecting patients, staff and partners with thevision and values of BCHC NHS TrustThey will use their expert knowledge and advanced clinical skills to assist in leading anddelivering safe and clinically effective, quality driven, personalised care for people withLearning Disability and the communities we serve.Working alongside the senior managers and clinicians, they will assist in leading services thatmeet the needs of service users and their families to achieve improved health outcomes andpromote health and wellbeing. Working in a multidisciplinary manner and across organisationalboundaries.

Main duties of the job

The role involves providing autonomous, evidence-based clinical care in line with professional codes, standards, and care pathways. The post holder exercises advanced clinical reasoning, decision-making, and accountability for managing a designated caseload, including assessing, diagnosing, and developing treatment plans. They are responsible for accurate documentation, communication with multidisciplinary teams, and maintaining high standards of clinical governance and risk management. Additionally, the role includes supporting junior staff, ensuring safe use of equipment, and escalating any concerns related to patient care or safety.
