Clinic-based position supporting nurse lead clinics across Locality - applicants must have an active NMC PIN number as essential to role.
Driving not essential. Flexible working hours required will need to be able to work on a Monday and Friday.
This is an exciting chance to join the East Locality to work as a Clinic, supporting non housebound patients that require clinical interventions that are deemed complex and do not meet practice nurse criteria. You will need to be able to travel to the clinic sites. You will provide a holistic approach to individuals, that includes, assessment, planning and implementation of care along with review. The successful candidate will act as an autonomous practitioner providing a comprehensive clinical assessment service to patients resulting in a safe and appropriate clinical decision.
The successful candidate should have experience of wound care, with a particular interest in leg ulcer assessment, treatment and follow up care including compression therapy and lymphoedema care as well as catherisations. Prescribing or a willingness to complete nurse prescribing essential. You will also support with triaging of referrals.

Main duties of the job
The post holder will work as part of the integrated Multidisciplinary team under the supervision of the Clinical Team Manager. He / She will be accountable for the delivery of high quality care to patients within health centers. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GP's, Tissue Viability and vascular clinics. You will support IMT triaging to ensure patients receive the right care at the right time in the right place.
