Ward-based - Hub Coordinator

We require enthusiastic and motived individuals to work as part of the Single Transfer of Care TeamHub. The role of a Hub Coordinator either on the ward or in the Hub itself is fast-paced, busy, with changing priorities, and we are seeking staff to join this highly valued team. Supporting effective patient flow across all Pathways for admission and discharge, your contribution to the work can be both rewarding and influential as the service continues to develop and evolve.

Main duties of the job

To work as part of a responsive, flexible team supporting the planning, facilitation and delivery of effective patient flow in the tasks associated with all referral management, admissions, coordination and support for simple and complex planned discharges across all inpatient areas at Birmingham Community Healthcare Foundation Trust.

To support the referral management across all Discharge to Assess (D2A) pathways, and the coordination and support for discharge planning and facilitation.

The postholder will be expected to work flexibly to ensure that service provision is maintained across 7-day service for all areas .

Job responsibilities

Main Duties

1. To support a comprehensive and responsive service to all inpatient areas/areas either at BCHC or other NHS Trusts, in relation to all aspects of bed referrals management, allocation, admission and discharge planning and facilitation.

2. To support the timely and accurate provision of data and information related to the admission and discharge of all patients with designated inpatient areas across a 7-day service.

3. To support the recording and management of referrals into the Single Transfer of Care Team - Central Hub related to patients who need to be considered for D2A bed allocation.

4. To actively liaise with patients and/or their relatives ensuring they are involved and informed regarding all aspects of their admission from the acute or community setting, and transfer to home, or alternative settings as part of their discharge planning and facilitation.

5. To liaise closely with all inpatient areas regularly updating on progress or changes in admission and discharge plans for any patient including appropriate escalation where relevant.

6. To maintain records related to the admission and discharge for all patients and actively contribute to other systems where relevant.

7. To support the process for clinical decision making as part of the D2A (Discharge to Assess) pathways alongside the Integrated Coordination Hub.

8. To monitor planned admissions via tools such as the Transport Portal, close liaison with the transferring inpatient areas, escalating as required any changes or potential delays.

9. To check all non-emergency patient transport requests are made in advance of the date of discharge wherever possible.

10. To record all requests for internal bed transfers and ensure the Integrated Coordination Hub is aware of these.

11. To liaise with inpatient areas ensuring a timely response is provided for all aspects of support with discharge planning, referring to the appropriate teams as required;

12. Communicate effectively and appropriately, demonstrating professional behaviour always;

13. To ensure accurate reporting of any patient who is recorded as Ready for Discharge or well enough to transfer to another setting as part of their discharge pathway, to the Integrated Coordination Hub.

14. Attend the daily ward-based MDT meetings as required to provide timely and regular updates with progress against discharge for patients the postholder is involved with and confirm all admissions have arrived as planned.

15. To ensure accurate reporting related to simple discharge to the ward nurse in charge and the Integrated Coordination Hub as required;

16. To develop a good relationship with ward staff, patients, relatives and other members of the team as required;

17. Contribute to team discussions as required, making suggestions for service improvements to the Integrated Coordination Hub Lead;

18. To contribute to the record of care using an appropriate standard of documentation on IT systems and related paper records;

19. To provide comprehensive verbal reporting to the appropriate MDT ward-based staff, the Clinical Team Leader, and Integrated Coordination Hub;

20. To maintain a high level of administrative organisation and documentation to provide a consistent level of information when not on duty.

21. To actively participate in discussions and provision of information specifically related to those patients who are deemed Ready for Discharge or whose discharge is delayed for any reason.

22. To undertake other duties commensurate with this grade of post in agreement with the relevant line manager.
