The main purpose of the Multi-Skilled Assistant is to deliver high quality, caring and compassionate patient care. The post holder will work across the disciplines of nursing, physiotherapy and occupational therapy, carrying out assigned tasks involving direct care as part of the Virtual Ward team. This role will involve working with patients as part of a multidisciplinary team to provide urgent care to ensure the patient remains in their home.

The post holder will undertake implementation and evaluation of patient care needs for a defined group of patients under the delegated responsibility and indirect supervision of a registered professional.

The post holder will be aware of their own competency and be willing to develop knowledge and skills to deliver high standards of care to patients.

The post holder will be expected to work shifts between the hours of 0800-2200 as part of a 7 day a week rota.

Main duties of the job

The post holder will provide support to the registered practitioners, undertaking clinical interventions commensurate with the role such as phlebotomy, venepuncture, taking observations and completing an ECG in preparation for a registered practitioner to assessor review.

The post holder will work alongside a registered practitioner providing support to 'double up' visits particularly for patients who have fallen and require two practitioners to assess and assist to a safe place.

Escalate and report any incidents that occur to a registered practitioner. Complete a datix following any incidents

Carry out appropriate joint visits with other members of the multidisciplinary team.

Follow the Lone Working policy and be aware of how to escalate any issues that may arise within the team in relation to this.

Never undertake anything beyond their agreed level of competency.

Ensure delivery of high-quality care in accordance with trust policies, procedures, and guidelines.

Accurately document all care delivered to patients, always maintaining confidentiality.

Participate in activities to improve the quality, productivity, and effectiveness of care in response to local and national policies and initiatives.

Adhere to infection prevention and control policies relevant to Virtual Wards.
