Job summary
BCHC is seeking an experienced, enthusiastic clinical pharmacist, ideally a prescriber, to join our Intermediate Care Pharmacy team.
You will plan and organise the provision of the pharmacy service within the intermediate care unit and provide professional advice and pharmaceutical supportThe post holder will have the opportunity to develop the medicines optimisation programme within the team in order to ensure the best quality care for our patients.
You will work as part of a multidisciplinary team to manage medicines on transfer of care from other providers, review patients' medication, prescribe within your scope of practice, plan for safe discharges as well as ensuring the secure and safe handling of medications in the setting.
Main duties of the job
The post holder will primarily work within the Intermediate Care units (ICU) working alongside the pharmacy technician, medical and nursing staff. The post is based at Ann Marie Howes Unit , but there may be an occasional requirement to provide cover at our other units or attend meetings elsewhere in the trust and so the ability to travel is essential.
Duties will include
- providing day to day guidance of work and support to the ICU technician
- Medicines reconciliation,
- level 3 clinical medication review
- assessment of patient's use of own drugs
- self- medication assessments and the counselling of patients for discharge
- prescribing and/or working with other prescribers to ensure safe practices and adherence to formulary
- plan for discharges
- ensuring supplies of medication to patients are maintained
- participating in audit
- training of pharmacy and other staff
- investigating or assisting in the investigation of any medication related incidents on the unit.
