Job summary
An exciting opportunity has arisen for Advanced Clinical Practitioners to join Birmingham Community Healthcare NHS Foundation Trust's Care Coordination Centre. We are looking to recruit positive, forward thinking individuals who are highly motivated, work under pressure and can manage complex care in the community setting.
Main duties of the job
The successful candidate must hold a Masters qualification in Advanced Clinical Practice. They will work as an autonomous clinical expert, providing clinical assessment, intervention and developing a holistic plan of care for the patient.
Core hours of the service are from 0800 until 22:00 currently, covering a 7 day service including weekends and Bank Holidays.
