Job summary
We are looking for a Senior Health Care support worker to join our experienced team within the Louth & Wold Community Nursing team
Located within the grounds of Louth Hospital the community nursing team covers a wide geographical area. You will visit housebound patients delivering wound care, medication, palliative care from a care plan under the direction of a registered clinician. This is a very rewarding job for anyone interested in supporting patients at home. You will work autonomously once induction has been completed ( pool car available) . Previous experience in community working would be advantageous. The team welcomes in site visits
Main duties of the job
The post holder will actively deliver care that meets the needs of patients at home or within a care setting under the direction of a qualified Nurse You will working within the Trust's policies and procedures at all times.
The post holder will record patient information and carry out a range of delegated clinical health care duties including nursing care ensuring that patients /clients receive safe, individualised and effective care which supports health and wellbeing and promotes self- care where appropriate and possible.
The post holder may also be expected to perform a range of non-clinical duties as requested including ordering of stores, audits, and attendance at allocated meetings.
