Job summary

Here in the Discharge to Assess Pathway 1 Team we have fantastic opportunities for Assistant Practitioners looking for the next step in their career in one of our 4 teams (Boston/Spalding, Sleaford/Grantham, Lincoln/Gainsborough & Horncastle/Skegness).

We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technician, Lead Assistant Practitioners, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants.

In our Pathway 1 Team, we follow the patient home and see them within 2 hours of their discharge. We listen to the patient of what matters most to them with regard to their personal and social circumstances, their physical, mental and emotional health and their carer's needs. This will include exercise programmes for their physical and mental wellbeing, recommending and providing the correct equipment, proactive work with patients to encourage independent living, educating patients on self-management of their health and wellbeing such understanding frailty symptoms and the importance of staying well physically and mentally for falls prevention, and so much more.

Being part of our Team, you will be supporting assessments and reviews within a given Team, but also closely working with colleagues across other teams within the service.

You will be working in a patient centred manner, enabling individuals to regain independence with care needs and meeting therapy goals.

You will form part of the service's leadership team, supporting patient planning, staff management and service development.

Qualifications

Essential

  • Assistant Practitioner degree or equivalent
  • Relevant clinical knowledge acquired through training

Desirable

  • Moving and handling training

Skills/competencies

Essential

  • Ability to prioritise own workload, including dealing with non-routine and the unpredictable nature of the workload.
  • An understanding of the scope of the role of the assistant practitioner in context of the team and the organisation, and how the role may contribute to service.

Knowledge & Experience

Essential

  • Experience of working with adults within health and social care.
  • An understanding of rehab and promoting independence.
  • Basic IT literacy.

Special attributes

Essential

  • Ability to work without direct supervision but with the guidance of a registered therapist.
  • Ability to travel as service delivery will be completed in areas not accessible by public transport.
