Job summary
Due to our ever developing and expanding therapy and Allied Health Professionals offer here at Lincolnshire Community Health Services we are now able to offer a Physiotherapy rotation in the South West of the county.
With the development of the Grantham Frailty Assessment Unit (FAU) we are able to offer rotations in this part of the county for the first time. The Grantham FAU will offer same day chair based care to deliver Comprehensive Geriatric Assessment as well as an inpatient rehab setting.
These posts are ideal for newly qualified or existing Band 5 Physiotherapists wanting to gain experience of community working within areas such as Adult Therapy, Community Hospitals, Transitional Care, Lincolnshire Stroke Service and Cardiac Rehabilitation . Our staff see people in their own homes, care homes, community rehab wards and clinics, working with individuals to support them to be the best they can be; following the Home First principles and providing care closer to home
Main duties of the job
The teams offer specialist holistic therapy assessments and intervention with the aim of maximising function and independence post-stroke and therefore reducing the requirement for care services. Care planning and patient-centred goal setting is a fundamental part of the person's journey and integration with social care teams, nursing teams, and other stakeholders is essential. The teams also have a proactive approach with emphasis on self-management of conditions, and empowering people to live well for longer.
As a band 5 Physiotherapist, you will have the opportunity to assess people with a wide variety of conditions such as post-surgery and post-stroke rehabilitation, falls and frailty and deteriorating long term conditions e.g. Parkinson's Disease and COPD on your rotations.
