Job summary

This is a pivotal role in the core neighbourhood team working as part of an integrated workforce across the East Lindsey and Meridian Medical Primary Care Network areas. Specialist Neighbourhood Practitioners are highly experienced and skilled clinicians with excellent leadership skills, working on behalf of the Lincolnshire health and care system to provide clinical oversight of people referred into Neighbourhood Teams. They provide leadership and develop relationships with stakeholders from across the health care and support system to ensure some ofmost complex and vulnerable people in our communities receive coordinated personalisedcare from relevant system stakeholders.

Main duties of the job

In this role you will:

  • Provide clinical leadership to the core neighbourhood team andact as chair of multi-disciplinary neighbourhood meetings.
  • Coordinate complex care and provide a vital link between primary secondary and specialist services.
  • Undertake comprehensive assessment of people with complex needs where there are gaps in or people are disengaged from other commissioned services
  • Identify people at risk of unplanned hospital admissions, people housebound or with complex needs requiring coordinated cross system interventions
  • Pro-actively engage with those people, their informal carers and those involved in their care to put multi-agency and person-centred care and support plans in place aimed at reducing admissions
  • Pro-actively engage with secondary care and community in-reach teams to reduce length of stay in hospital
  • Pro-actively engage with domiciliary and care home providers and partners across statutory and voluntary sectors

Job responsibilities

  • To provide advanced clinical care using high level assessment to make a diagnosis, initiate / prescribe medical / nursing treatments, refer appropriately and co-ordinate the patients journey across the health, social care and voluntary sector system, regardless of care setting
  • Be able to perform comprehensive geriatric assessment (CGA) and work toward strength based care
  • To work as an accountable and autonomous practitioner working to pro-actively monitor and anticipate changing phase of illness and health and social care needs of patients with long term conditions and / or complex presenting care needs
  • To ensure delivery of a person centred approach, based on What matters to me to all patients that enables individuals to achieve their optimum physical, psychological and social well being
  • To provide a timely response to referrals and be responsible for advanced clinical care using comprehensive, evidence based, holistic assessment
  • Promoting high quality standards and cost effectiveness in all aspects of care delivery.
  • To be responsible for effective feedback from all members of the neighbourhood and integrated working teams
  • To be the designated clinician for named GP practices in a geographical area and be responsible for maintaining comprehensive clinical communication with general practices and neighbourhood colleagues, providing timely feedback to the service manager
  • Improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home / community and reduce the need for, or prevent inappropriate hospital admission
  • Identification of high intensity users with needs using risk stratification systems and other sources of information

Essential

  • Registered Health Care Professional holding current registration with appropriate regulatory body.
  • Evidence of working to PGDs with advanced knowledge (AHPs)
  • Safeguarding-level 3 or willingness to undertake
  • Minor Injuries level 6/ or 7 or very relevant experience
  • Minor Illness level 6 or relevant experience

Desirable

  • Non-Medical Prescribing qualification

Previous Experience

  • Evidence of recent and on-going personal development including obvious leadership development
  • 2 years post registration experience in relevant urgent or emergency care settings
