Job summary
The Discharge 2 Assess Pathway 1 Service was created to ensure any person in a Lincolnshire hospital who is able to return straight home from hospital does so. It is then important they have immediate reablement and rehabilitation so they are able to live as independently as possible into the future.
The purpose of the service is to ensure people are assessed for their long term needs in their own home wherever possible. This will reduce their length of time in hospital and avoid further deconditioning and low mood.
The role of the registered therapist in the service is to carry out these assessments for longer term needs and coordinate a comprehensive rehabilitation care plan to maximise independence. It is also to provide leadership to the therapy assistants in the service. In addition as a newly developing service there is a substantial chance for service development and forging relationships across the spectrum of therapy and care providers in the county.
Main duties of the job
The Pathway 1 Discharge to Assess Service provides therapy based rehabilitation and reablement to patients immediately following hospital discharge. It will be the role of the therapists to provide profession specific assessments, create rehabilitation care plans and to work closely with the team of Advanced Therapy Assistant Practitioners, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants to ensure patients are seen and reviewed in a timely manner to maximise recovery. To work closely with all members of the multi-disciplinary team and to work at times in conjunction with other agencies to determine a person's long term care needs.
The ideal candidate is not just a person who is absolutely focused on maximising each patients independence but also ensures that happens at home wherever safe to do so. They will be a good networker and innovative problem solver. Successful candidates for this post won't only be experts at working with patients but also able to value and develop support workers.
