Job summary

An exciting and rewarding opportunity has arisen within the Community Cardiology Team for one new Heart Failure Specialist Nurse to join our Community based Heart Failure Service.

We are seeking a driven, enthusiastic and innovative individual with an interest in the care and management of patients with Chronic Heart Failure to join our dynamic team.

The Community Heart Failure Team is engaged in system wide change, having strong supportive networks across the Integrated Care System and are committed to transforming the way that Heart Failure care is delivered which is patient focused and closer to home.

The team has a friendly and supportive work ethic. "Huddles" are arranged daily to discuss clinical cases/clinical supervision with a fortnightly virtual MDT meeting with Cardiologists from secondary care.

Main duties of the job

You will be supported to manage a caseload of patients with highly complex or unstable heart failure needs in their homes or other community settings in either face to face, telephone or video consultations.

You will work with patients to formulate individual self management plans, enabling people to remain in their own home and prevent avoidable hospital admissions. You will use advanced clinical assessment skills to diagnose and treat signs of instability, optimise heart failure therapy and sign post to other health care professionals where indicated.

The successful applicant must have access to a vehicle for work purposes. This is a countywide role so applicants should expect to travel. Geographical area and location of patient caseload will be located within South Lincolnshire covering areas including Boston, South Holland and Welland.
