Job summary
As a specialist occupational therapist working within Community Hospitals you will have the opportunity to champion the key values of your profession.
We understand that through purposeful and meaningful activity we can support people to rehabilitate, recover and self-care. Although your base will be on the ward, we understand that working beyond boundaries is the only way to holistically support people toward their goals; within this role you will have the opportunity to embrace the 'discharge to assess' ethos by working alongside our partnership organisations in order to support individuals safely home.
We strongly believe that our therapy teams within our community hospitals play a vital role not just in the health and well-being of our patients, but for our wider communities as a whole. By working proactively to identify current and possible future needs we can empower people to manage their own care, so looking to promote their health and wellbeing and reduce their future dependence on services as a whole.
As your caseload will be varied, you will have the opportunity to work alongside individuals experiencing a range of long-term health conditions. A key aspect of this will involve assessment of frailty and providing meaningful support for people with palliative health conditions.
This role is to be based on Archer Ward at Louth County Hospital, LN11 0EU.

Main duties of the job
- To assess and develop personalised care plans for individuals admitted on to the ward
- To (where appropriate) complete/delegate visits to individuals homes
- To provide clinical leadership and supervision to members of the therapy team
- To work with partner organisations in order to promote a 'discharge to assess' model of care
- To work alongside the therapy leads in order to innovate and develop the therapy service
- To demonstrate a commitment to personal development
