Job summary

Sleaford Community Nursing Team are seeking a full time Case Manager/ District Nurse to join the team.

Sleaford is a thriving market town and civil parish in the North Kesteven district of Lincolnshire.

Sleaford integrated community team is a very friendly team covering a large rural area, delivering care to a diverse range of patient.

For the right candidate we would potentially consider someone without the DNSPQ qualfication allowing the opportunity of a developing case manager role.

Main duties of the job

The District Nurse/Case Manager will hold a caseload of patients with varying health needs, coordinating care delivery. They will need to have extensive communication, interpersonal and clinical skills in order to be successful.

The District Nurse/Case Manager is not only integral to leading on patient care but leading the community nursing team through challenging times and the transformation of services, role modelling excellent behaviours which are in line with our trust values. The Case Manager is essential to improving the quality of care that we deliver and in ensuring that the patient has a positive experience.

  • Mentoring or Assessor/ Supervisor qualification

  • Management or leadership qualification

Experience

  • Experience of working in the NHS or related multi-professional setting dealing appropriately with key partner agencies
  • Post registration experience in a relevant clinical setting
  • Experience of autonomous decision making/working

  • Experience of leading on changes within a team
