Job summary
Job overview: Lincolnshire Community Health Services (LCHS) are developing their delivery of IV Therapy to patients within their community, such as on the community wards, care homes and in patient's homes. They also have a working partnership with the acute trust to provide IV service to patients in their own homes or community settings reducing bed occupancy in the acute areas.
This is an exciting opportunity open to be involved in a newly established and fast-growing service within LCHS and is open to all registered practitioners who are interested in the delivery of IV Therapy in a community setting, both newly qualified and experienced registrants.
A bespoke robust training package has been developed where you will have the opportunity to work closely with members of the IV team as well as completing any necessary additional training for the role with our other service providers.
The job covers 7 days a week working, with full and part time posts being considered.
This teams aim is to provide care to patients within their own homes or in community settings to prevent acute hospital admissions, help ease bed capacity within the acute hospitals and to deliver care closer to home for patients.
Teams are cross county working being based in three community hospital sites across the county, Gainsborough, Skegness, and Spalding.
The current vacancies are for either Skegness, Lincoln or Gainsborough sites.
Teams will be line managed by a band 6 therapy Clinical Team Lead.

The role will involve working in a small community team delivering care to patients in their own home or community settings. You will work in partnership with patients, carers and other health professionals to promote health and social wellbeing developing individualised care plans for each patient.
You will be accountable for the daily delivery of IV Therapy through a CVAD device; interventions will include assessment, treatment, and evaluate your patients care, observations, drug calculations, blood taking, dressing, and recording visits and care delivered in a patient electronic record.
