We are pleased to offer an exciting opportunity for a proactive and highly organisedProgress Chaser to join our Hospital at Home team at Abel Smith House in Stevenage. This rewarding role is central to supporting patient flow, ensuring individuals receive timely, safe, and well-coordinated care throughout their inpatient journey.
As a key member of the multidisciplinary team, you will play an integral role in progressing patient pathways and helping to achieve Expected Discharge Dates. You will work collaboratively with clinical colleagues, patients, carers, and partner agencies to coordinate actions, resolve delays, and support smooth, efficient discharges. Your ability to communicate effectively and build positive working relationships will be essential in delivering high-quality, patient-centred care.
Main duties of the job
This position offers a varied and engaging workload, where strong organisational skills and attention to detail are vital. You will be responsible for maintaining accurate records, updating patient systems such as SystmOne, and supporting the coordination of services including therapy, nursing, pharmacy, and social care. You will also contribute to team meetings, audits, and continuous service improvement.
