Main duties of the job
Main duties of the job include the following:

Conduct comprehensive assessments and deliver evidence-based speech, language, communication, and swallowing therapy to adults with neurological conditions in both community and inpatient settings

Work collaborativelyas part of a multidisciplinary team to deliver integrated, person-centred care.

Develop and implement individualised treatment plans in collaboration with patients, families, and the multidisciplinary team.

Provide specialist clinical adviceand support to patients, families, and carers to enable informed choices and effective involvement in the rehab process.

Supervise, mentor and supportjunior staff, assistants, and students to foster learning and maintain high standards of practice.

Contribute to service development, quality improvement initiatives, and audits to help shape and improve the delivery of the service.

Maintain accurate, timely clinical recordsand contribute to care planning, discharge planning, and onward referrals.

Promote self-management, empowering patients to build confidence and sustain progress beyond formal rehabilitation.

Participate in regular team meetings, in-service training and CPD, keeping up-to-date with the latest evidence and clinical guidelines in stroke care and community rehabilitation.
