Are you looking for a new challenge and interested in working with children and young people with complex physical health needs in the community? If so, we are recruiting a Children's Complex Needs Co-Ordinator. The post will involve working with children and young people attending the four special schools across West Hertfordshire.

You will deliver a specialist nursing service for children and young people with complex health needs attending these special schools. You will work within the Special School Nursing service to provide year round support, working in partnership with the children, young people, and their families and carers, together with other professionals and agencies to meet health, development and safeguarding needs. You will champion the needs of children and young people with disabilities, building effective working relationships across organisational boundaries. To ensure the voice of the child is heard, ensuring you are acting as an advocate for the Children, Young People and their families.

Main duties of the job

The position requires an experienced Registered Children's nurse who is passionate about meeting the health needs for children and young people with disabilities in a way that maximises their opportunities.

This role will support Children and Young People on the West Hertfordshire Special School Nursing caseload with complex physical health needs requiring multi-agency involvement.

Acting as an expert resource providing support and advice as required to professionals involved in the care of the Child or Young Person with complex needs.

To liaise, co-ordinate and build effective working relationships with colleagues from across a multitude of agencies to promote seamless communication and collaboration between services.

To be a point of contact outside of term time providing advice and support for professionals supporting with the monitoring of ongoing health needs or managing acute deteriorations in health such as the Children's Community Nursing service. To ensure seamless communication through the provision of a handover of significant updates to the Special School Nurses at the start of term.

To support Children and Young People to return to school following on from episodes of prolonged illness through attendance at discharge planning meetings, multi-disciplinary team meetings and where appropriate, home visits to assess potential changes in health needs which will impact on their ability to attend
