Job description

Job responsibilities

Daily the team meet to allocate patients, where you would be included. You would have patients to support guidance from a registered practitioner. You would also support the team with admin tasks, which could include referring to other services, equipment ordering and office tasks amongst others.

The main aspect of the role is clinical. Supporting the team by visiting people at home. This may include reviewing a patient with a team member, providing equipment after a colleague has assessed, helping a patient relearn how to do a functional task with guidance or helping a patient to complete their exercise programme provided by a colleague, amongst other tasks.

You would be expected to follow the trusts organisational and service policies and procedures whilst working within legal framework. Eg Consent.

You would be expected and supported with completing competencies that will help guide your practice and you will have a supervisor and buddy as part of the trusts support for new starters.

We would provide you training relevant to your role and would expect you to take an active role to support your learning. This is expected to be kept up to date as per trust policy.

You will have an active role in the team meetings and in-service training and will have access to functional assessor training amongst others. Included expectation is for you to be an active member within our multidisciplinary team meetings that support our patients with coordinated care.

  • Experience of working with people in the community.
  • Good communications skills both written and verbal.
  • To be able to work under pressure and be flexible.
  • Secondary level education to GCSE/O level or NVQ 3 or equivalent with experience of working as a Rehabilitation Assistant with demonstration of related competencies
  • Previous experience or knowledge of health & rehabilitation.
  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
