We have a vacancy in our team for a Band 6 Community Specialist Palliative Care nurse.
You will be joining a small team committed to managing palliative and end of life advice and support to patients and their families with complex needs who have life limiting illnesses.

Main duties of the job
The successful candidate will:
- Be able to work autonomously as well as being an active team member
- Be able to conduct a holistic assessment including symptom management discussions
- Be part of a patient focused team, working collaboratively with Integrated Care Teams, GP's, local hospices, and acute care colleagues.
- Work in partnership with family support therapists and psychologists to deliver complex psychological support.
- Attend monthly clinical supervision sessions to support practice
- Be experienced in using electronic records whilst mobile working (though support will be offered with this)
- Be offered a wide range of training opportunities including advanced communications training, psychological assessment skills and career progression coaching.
- Be flexible to meet the needs of the service
- Have a full valid driving license and access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010)

- Provide specialist palliative care advice and support to patients and their families who have life limiting illness and reside in North Hertfordshire, Stevenage and Royston areas, 7 days a week (including band holidays) 9-5pm. This could be in a clinic, virtually or in the patients home.
- Provide specialist palliative care advice and support to generalist nurses, care agencies, health and social care professionals including doctors and consultants, to ensure high quality individualised patient care. This will include support with education and training in conjunction with the band 7's in the team.
- Participate in triaging new referrals to the combined specialist palliative care services across East and North Hertfordshire, based at the integrated Care Co-ordination Centre at Abel Smith House in Stevenage.
- Work towards enhancing your palliative care skills including symptom control with the support of the band 7 team members.
- Work independently as an autonomous practitioner and as a member of a multidisciplinary team whilst being aware of your personal well being and professional boundaries.
- Maintain confidentiality at all times and manage information sensitively.
- Demonstrate discretion, dignity and respect when communicating with patients, colleagues and other agencies.
- Act as the patients advocate whenever necessary.
