We are looking to appoint enthusiastic and hardworking Care Co-Ordination Administrators for our busy community team. You will provide confidential and efficient administrative support to patients and services, including Hospital at Home.
This is a busy and fast paced role which requires good interpersonal skills and a positive attitude.
You will be required to work on your own initiative and demonstrate a willingness to help others and will be a key member of the team. Excellent interpersonal and communication skills are essential combined with an ability to deal with sensitive and potential challenging situations.
You must be able to build effective working relationships, be approachable and with a flexible approach to work.
Enhanced payments for weekend working with a day off in the week.
Main duties of the job
The candidate will be expected to be able to prioritize work and meet deadlines.
Duties will include: -
- Undertaking a range of administration tasks, including processing referrals, letters and emails
- Answer incoming calls, dealing with inquiries/referrals from patients, relatives and professionals.
- Liaising with GP's/Nurses and Therapists.
- Ensure accurate and relevant information is recorded on our clinical system database.
You will receive full support and training to undertake all aspects of the role, you need to bring enthusiasm, energy and a flexible approach to work.
The shift pattern will comprise of alternate weeks of 8am-4pm and 12pm-8pm. You will also be required to work either Saturday or Sunday once a month.
