Job summary

An opportunity has arisen within our Children and Young People's Specialist and Developmental Services department for an Advanced Clinical Practitioner Nurse (8a) or a Trainee Advanced Clinical Practitioner Nurse (7).

Successful candidate will be based at one of our centres in either Watford, Hemel Hempstead or St. Albans. Base negotiable.

The post holder will apply advanced clinical expertise, levels of judgement, discretion and decision making within the in-patient units. Will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with complex presentations and multi pathologies.

Trainee:-

During the training period of this post, the post holder will increasingly learn to apply advanced clinical expertise, levels of judgement, discretion and decision making within the Community Paediatrics team, this will involve placements across all Hertfordshire Community Trust Child Development Centres, as well as other relevant healthcare environments.

During the time as a Trainee Advanced Clinical Practitioner the post-holder will work under decreasing supervision & with increasing independence to act as an autonomous practitioner, exercising expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with complex presentations and multi pathologies.

Main duties of the job

The post holder will hold a caseload of children and young people within the Community Paediatrics Service participate in the assessment and review of children with neurodevelopmental and neuro disability conditions in line with established and agreed pathways. The role will provide an expert resource, providing a visible clinical profile, having both advisory and clinical input into patient care within the Community Paediatric Team. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. Will work in partnership with the Community Paediatricians to lead the development of the nursing offer/nurse-led pathways within the Community Paediatrics service.

Trainee:-

The post holder will work towards holding a caseload of children and young people within the Community Paediatrics Service participate in the assessment and review of children with neurodevelopmental and neuro disability conditions in line with established and agreed pathways.
