Job summary

Are you a nurse who has specialised in cardiology, with a particular interest in heart failure? This new role for the Lead Clinical Nurse Specialist in the Integrated Heart Failure Service is a rare opportunity to not only to lead a service, but to be part of the development of it too. The Integrated Heart Failure Service is a new service within Hertfordshire Community NHS Trust (HCT), working across East and North Hertfordshire. The service works in an integrated, joined up and coordinated way with primary and secondary care services.

This role has overall responsibility for the function of the team.

  • Providing visible leadership to the team
  • Ensuring that all processes and standards are met
  • Coordinates the assignment of HF case managers and manage the cohort of patients
  • Provide clinical expertise in the management of HF patients
  • Provide advice and support to junior staff in management of clinical risk.
  • Prescribing medications in line with PGA

You will work collaboratively with a range of people including senior colleagues and clinicians in secondary and primary care.

You will engage with a range of colleagues from partner organisations to develop and lead this new integrated heart failure service across East and North Hertfordshire.

You be part of the recruitment of a team of nurse specialists and unqualified staff and then lead this team

You will be responsible for ensuring a high quality service that is cost effective at the same time

Main duties of the job

Service management and leadership

  • To have overall responsibility for the leadership of Integrated Heart Failure Service, providing both the operational and clinical leadership
  • Lead a skill mixed team delivering the heart failure service
  • Work in partnership with primary care, cardiology consultants and specialist nurses in secondary care, hospices, Hospital at Home , the integrated care nurses working in the community, the voluntary sector and well being services in East and North Hertfordshire.
  • Manage the budget for the service
  • Work collaboratively with the operations manager and senior nursing team to ensure achievement of key performance indicators and improvement in patient outcomes.
  • Be responsible for the development and provision of patient pathways and guidelines in line with national standards

Provide expert nursing practice

  • Demonstrating a high level of advance clinical competence, based on in depth knowledge, research and evidence-based findings.
  • Be responsible for a patient case load as an autonomous practitioner with specialist skills.

The role will require the ability to work in collaboration with senior colleagues, clinicians, technical subject matter experts, frontline staff and engage with a range of colleagues from partner organisations to develop and lead the new integrated heart failure service across East and North Hertfordshire. .

The post holder will manage and lead a specialist integrated heart failure service with responsibility for ensuring high quality, cost-effective healthcare or manage and lead a workforce of specialist nurses and unqualified staff.
