Job summary
We are seeking an Advanced Clinical Practitioner, who has a passion for working with children, young people and families,to be part of the two CAMHS Crisis and Home Treatment Teams (CAHTT) across Herefordshire and Worcestershire.
The post holder will be a Registered Healthcare Professional,an independent Non-Medical Prescriber and hold an MSc in Advanced Clinical Practice (or equivalent at MSc level to be agreed as part of the application process).
They will demonstrate indepth knowledge and competence in all aspects of urgent mental healthcare within the child/young persons home setting & environment, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient-centred care.
The service operates 0800-2000, 7 days per week including bank holidays and the successful applicant will be expected to work flexibly Mon-Fri, and be able to meet the travel requirements of the role.

Main duties of the job
You will work in partnership with the service manager, delivering robust clinical leadership alongside their operational leadership and contributing to the development of this dynamic service.
You will work independently assessing, diagnosing and treating patients with urgent mental health needs in the community, making complex decisions and keeping collaboration and patient choice at the heart of what you do.
Alongside colleagues from the multi-disciplinary CAHTT teams, you will be lead in the assessment and treatment of complex patients. You will work closely with partners across Primary and Secondary Care to ensure the most appropriate input is delivered in the most appropriate location. We work in partnership with other organisations such as social care, education and acute healthcare, to support children and young peoples mental wellbeing in the most holistic of senses.
You will be delivering care in new and innovative ways-the role will be well suited to evidencing all four pillars of advanced practice and time will be provided to work on quality improvement.
Where applicable, you will be expected to share your expertise and knowledge with trainee ACPs and act as a supervisor/mentor.
You will be supported by the senior clinical team including trust doctors/consultants, clinical leads, pharmacists, psychologists and senior nurses. Each team holds daily, weekly and monthly meetings which you will be expected to attend.
