currently have an exciting opportunity for a senior community nurse to join our

You will be expected to work a combination of early (07:30 15:30), mid (8.30 4.30) and late (10:30 18:30) shifts as well as weekends. We are happy to consider flexible working requests.
Main duties of the job
We
are looking for an experienced, highly motivated, flexible team player with a
range of nursing skills. Excellent written and verbal communication skills are
essential.
As
a Senior Community Nurse you will be expected to work as a key member of the
multi-disciplinary team to assist in reducing hospital admissions, facilitating
hospital discharges, delivering end of life care and community nursing. Your
role will involve the assessment of patients holistic needs and the implementation
of robust care plans, alongside case management and allocation. You will be
given management and supervisory responsibilities for band 3 and 5 staff,
offering them guidance and support.
This
is a great opportunity for personal and professional development that will
expand your knowledge and experience within the Integrated care System. You
will be supported by the Band 7 Team Leader with regular supervision.
We
are a community-based neighbourhood team
providing care to patients in their own home. We aim to deliver an effective,
high quality service that puts patients at the centre of their own care. You
will work closely with family, GPs, Social Care, the Voluntary and private
sector, Community Therapists, Advanced Clinical Practitioners, working across
the planned and anticipatory, admission prevention, hospital discharge and,
rapid response pathways.
Due to the nature of the post the applicant must be able to travel independently.
