Job summary
We have an
opportunity for a fixed term contract or secondment until 30th
September 2024 for Senior Triage Clinician who has a passion for Urgent
Community Response (UCR) with a particular focus to support pathways to keep
patients in their usual place of residence in their last days of life. The post holder would be expected to work closely with the
Neighbourhood Teams and system wide colleagues to ensure the most appropriate
care and end of life pathways are facilitated. The post holder will also be
required to support in the general triaging of referrals received into the UCR
Hub with active participation in daily Teams calls with West Midlands Ambulance
Service to identify suitable patients to pull from the Ambulance stack.
The role will
involve regular in-reach within Worcestershire Royal Hospital A&E and SDEC
departments providing support and promotion of the UCR function of NTs with
particular focus on palliative/end of life support that can be offered in the
community and linking with relevant Acute colleagues. There will also be an
expectation for the post holder to assist with any training and support for
existing staff within the UCR Hub and Neighbourhood Teams. There will be an
expectation for the post holder to link closely with the Evening and Night
Nursing Teams including occasional shadowing an Evening triage shift.
You would be
expected to work shifts across 07.30-18.30 with regular weekend (currently on a
1:3 basis) and Bank Holiday working.
