Job summary
An exciting opportunity has arisen for a
part-time Sneior Mental Health Practitioner to join our Neighbourhood Mental
Health Teams (NMHT). The NMHTs are at the forefront of providing accessible
care, integrating both primary and secondary mental health services, in line
with NHS long-term objectives. There are 5
neighbourhood teams in Herefordshire. The post advertised is to work within
Hereford City Neighbourhood team. Each NMHT is comprised of Mental Health
Practitioners, Support workers, Administrators, Clinical Psychologists and a
Consultant Psychiatrist. Additionally, the teams are supported by our voluntary
care sector with embedded Mind, Link Workers and Vocational Specialists from Landau, with
the aim of providing a holistic, recovery-focused service. We work closely with
our Primary Care Networks, demonstrating good relationships and communication
with our respective GP partners. The teams are supportive, with regular MDT
meetings, Individual/group supervisions and team away-days. For more information,

Main duties of the job
The post holder will provide prompt local
assessment, triage, risk assessment, risk management, signposting, onward
referral and evidence-based interventions for people presenting with mental
health problems.
The post holder will offer advice, support and
training to other Mental Health Practitioners, Primary Care, VCS and other
Mental Health/ Community Teams and promote recovery and social inclusion.
The post holder will work collaboratively with
internal and external organisations and key partners to meet the service needs
of the patients, carers, clinicians and other practitioners within the PCN
area.
The
post holder will attend team and internal and external service wide meeting
with the occasional role of chair.
The
post holder will have experience of working within a mental health setting
within a senior role.
The
post holder will be required to complete statutory and mandatory training as
required for the role, and keep this updated as required
The
post holder will have the ability to travel countywide and are required to have
business insurance.

- Provide a presence for consultation for all Mental
Health Practitioners, primary care, VCS staff and any other stakeholders
involved in the patients care within the ICS.
- Represent the team on specific projects and working
groups as required
- Working within a locality service and through
participation in the local referral and care pathway, provide clinical
assessment and appropriate triage with a view to risk assessment, risk
management, safeguarding, sign posting, treatment or onward referral in
response to identified patient need.
- Work closely with others, providing advice and
guidance, where required, along with a more in-depth specialist holistic mental
health assessment if this is indicated. This will include a care plan, linking
patients into additional services, if required. The aim being to support people
to address issues that may be impacting on the effectiveness of the therapy
that they are receiving.

should this be required, for patients with severe mental health problems that
have now become stable.
- Ensure that
information and progress regarding patients are communicated promptly and
accurately to other team members and relevant organisations whilst having due
regard for confidentiality.
