Main duties of the job
As a community Nurse

unplanned nature. The service works as an on-call service covering the County
of Worcestershire taking direct referrals from patients, 111, Ambulance Service
and discharging hospitals. You can expect to have a comprehensive induction and
work alongside peers until you are confident to then work alone.
The evening service nurses primarily work alone, and have
work allocated via a handheld tablet by the triage nurse based at Droitwich.
You will be able to start your visits from home if the geographics dictates.
The
patients that are seen during the evening and night vary immensely and no 2
shifts are the same. For example, we see catheters, palliative care, wound
care, administration of medication, diabetics, and patients requiring intensive
support rehabilitation following discharge from hospital or admission prevention.
As a team we often undertake initial assessments of patient in the community.
As part of a new initiative we are working
closely with West Midlands ambulance service and 111 and assist in responding
to urgent Care response. These can be patients that have fallen, or patients
requiring further assessment to aid prevention in admission to hospital.
