Job summary

The Evesham, Broadway, Bredon and Inkberrow (EBBI) Neighbourhood Team is a friendly and hardworking team who are looking for an enthusiastic healthcare assistant to join us at our base in Evesham.

The Neighbourhood team is looking to recruit someone who is happy to work across our whole patch on a full-time basis (37.5 hours) who is keen to join the team in supporting us to deliver high quality care.

We are looking for a passionate individual who is excited by change and keen to help shape a new way of delivering community integrated care. We work closely with our GPs and work hard at developing relationships with primary care services, drawing on the skills of our MDT colleagues to manage complex patients and ensure we are responsive, and patient centred.

Main duties of the job

  • While on duty you will either be working alone or alongside colleagues to support delegated tasks in line with Trust standards and core values.
  • Each task will have been assessed by a qualified Healthcare professional and a care plan will be in place for you to follow and report back on.
  • Tasks may include personal care, supporting daily activities, participating in manual handling and other allocated tasks such as dressing wounds, setting up equipment, and recording observations.
  • All clinical documentation is recorded in line with Trust guidelines, and you will give daily handovers to team leads summarising your involvement with your allocated patients.
  • Clear and timely Communication is key to your role which may be in a verbal, non-verbal or written format to support patient centred care.
  • There are three core working shifts that cover the service from 7:30am 6:30pm Monday to Sunday.

  • Experience of working with older adults in a health/social care setting

Desirable

  • Community experience of older adults with physical and/or mental health problems.
  • Experience of working and liaising with staff from different disciplines

Qualifications

Essential

  • Level 3 qualification (e.g. NVQ 3, A-level) in Care or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post
  • Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer

  • Intermediate Skills Programme

Knowledge

  • Basic knowledge of physical/mental health conditions
  • A level of understanding of rehabilitation
