improving the quality of life for our stroke survivors. We consider our staff
to be our most valued asset and are committed to staff wellbeing, opportunities
for flexible working and staff development.
We are working towards implementing
the National Model for Integrated Community Stroke Services. This brand-new
role is an exciting opportunity to be involved in service design and
improvement, offering SLT rehab programmes to all stroke survivors in
Worcestershire including our in-patient rehabilitation unit and community
stroke provision.
The role is 37.5hrs over 5 days in leading the
team of Speech and Language Therapists across the community stroke pathway and
working in partnership with our acute colleagues to facilitate safe transfers
of care.
The ability to use your initiative, work proactively, demonstrate excellent communication and influencing skills, work within a team, manage and prioritise your own workload and respond flexibly to changing demands are essential requirements of the post.
Main duties of the job
You will lead the team of SLT stroke clinicians,
prioritising the integrated stroke caseload and working in partnership with the
acute SLT team. You will be responsible for the clinical supervision, education
and support of the SLT team and in collaboration, with the other stroke Leads,
help to strengthen and develop the stroke pathway.
Alongside your SLT colleagues, you will be undertaking
comprehensive assessments and treatment of stroke patients with speech,
language, voice, fluency and/or swallowing disorders. You will formulate
individual programmes of rehabilitation using clinical reasoning and utilise a
range of treatment skills, targeting realistic and measurable, patient-led goals.
It is also expected that you will offer support and education to carers/families
and the wider team.
As we develop the stroke pathway, you will be working in
all settings, including community hospitals, care homes, patients homes and
clinic settings.
