Job summary

A new opportunity for a Senior Community Mental Health Practitioner has arisen within the Bromsgrove Neighbourhood Mental Health Team. This is a newly created post and will be for 12 months as fixed term or a secondment opportunity for staff already employed within the Herefordshire and Worcestershire Health and Care NHS Trust. This staff member will have experience working in a community setting as a Band 6 clinician and managing a complex, mixed caseload. They will be able to complete highly specialised assessments and interventions on complex individuals, particularly those who have severe and enduring mental illness, a history of substance misuse issues, other co-existing diagnoses or have a forensic history. The successful candidate will be a role model for junior staff within the team, and be able to provide them with support, education and guidance with their level of expertise and knowledge.

Main duties of the job

The job role entails (but not limited to):

  • Provide front line assessments to highly complex individuals and be able to care plan and risk management these patients.
  • Provide specialised evidence-based interventions to patients and be able to determine appropriate needs led plans of care.
  • Work as a key member and resource of the MDT core team.
  • The ability to work flexibly as the need of the service demands is essential.
  • Provide a presence for consultation and support junior members of staff with complex cases and provide support in formulations
  • Liaise with internal and external services such as the police, drug/alcohol services, CNS, EIS, VCS, PCN stakeholders.
  • Have a good working knowledge of the Mental Health act 1983 and Mental Capacity act 2006
  • Communicate highly sensitive and/or emotive information to members of the public.
