Community Nurse

The closing date is 01 April 2026

Job summary

An exciting opportunity has arisen for a Community Nurse to join Riverside Neighbourhood Team in Worcester. Within the community nursing role, you will reap the rewards of being an autonomous practitioner to deliver individual patient care. This will give you the rewarding opportunity to provide holistic care to patients.

Shift patterns include 07.30-15.30, 08.30-16.30, and 10.30-18.30. Flexible working opportunities are available, and we would welcome you to discuss these options at interview.

The Neighbourhood Team consists of Community Nurses, Physiotherapists, Occupational Therapists, Healthcare Assistants, and admin staff with the wider multidisciplinary team. We are looking for a highly motivated, flexible team player with a strong, quality driven patient focus and an ability to work through change to embed new ways of delivering care. In return we provide you with compassionate leadership within a supportive team environment and an individual induction package.

You will be fully supported to gain new skills and experience to deliver high quality nursing care to patients in their own home, clinic, and residential settings. You will also have the opportunity for professional development and progression. You should have excellent clinical, written, and verbal communication skills. We would love for you to join us in this transformational community care journey within the Worcester Neighbourhood Team.

Main duties of the job

A typical day as a community nurse may include a variety of skills such as the holistic management of diabetic patients, wound care, catheter care, IV administration and supporting patients at the end of their life. This is not an exhaustive skillset to the role and you will benefit from a diverse variety of skills and scenarios enhancing your career development.

The Neighbourhood Team responds to urgent care response calls generated from a variety of referrals including West Midlands Ambulance Service. We also assess and provide care to patients to prevent hospital admission and support hospital discharges.

No day is the same within the community and each day is as rewarding as the next, whether you have healed a leg ulcer or given a patient the best possible care at the end of their life. One thing we promise you is that community nursing is never boring!
