We are excited to be recruiting into the new role, to lead and coordinate the Gold Standards Framework into Community Hospitals across Worcestershire, working collaboratively with colleagues in Worcester Acute NHS Trust and Wye Valley NHS Trust.

We are looking for someone to work 37.5 hours per week. You will be based with the Community Specialist Palliative Care Team in the Community Hub at the Princes of Wales Hospital, but will be working closely with the Community Hospital wards and Matrons, across Worcestershire.

Main duties of the job

The successful candidate will be leading and coordinating the implementation of GSF into community hospitals, your role will require you to work closely with the Matrons, wards managers, Advanced Care Practitioners (ACPs) and ward staff to support them, through formal teaching and working alongside to help them gain the skills and knowledge they require to implement this exciting programme. You will be supported by the EoL Educator who sits within the Specialist Community Palliative Care Team, Consultants in Palliative Care from across Herefordshire and Worcestershire, fellow EoL Care Facilitators who are based at the acute Trusts within the ICS, Matrons and staff within the Community hospitals. As this is a new role you will be expected to work within a community of practice virtual team made up of EOL Care Facilitators across the ICS. Together you will develop and lead the project/ resources required for system transformation. This project is being launched across Herefordshire and Worcestershire ICS and travel to Herefordshire on occasions will be required and across Worcestershire on a regular basis.
