Job summary

An exciting opportunity has arisen for a Band 7 Operational Lead in our Droitwich Ombersley & Rurals (DOR) Neighbourhood Team. This is a 22.5-hour post, to be covered over 5 days.

The successful candidate will work closely with our well established, friendly and supportive team whilst also working closely with other senior leaders across DOR and county wide. Operational leadership of the Neighbourhood team includes a focus on workforce, performance, quality, service improvement and oversight of a daily safe delivery of service.

You will work closely with the B7 Clinical Lead, Band 7 Physiotherapy Lead, B7 OT Lead and Admin Lead and be supported by the service lead. The post will also involve deputising at senior management meetings and working closely with the other Neighbourhood Operational Leads.

The role involves line management responsibilities for direct reports consisting of Band 6 Senior Community Nurses and a senior Administrator. Managing the staff roster to ensure that all shifts are covered is a key part of the role as well as organising bank and agency staffing where necessary. You will work closely with the Integrated Service Lead to ensure that recruitment is managed within budget and HR targets and service targets are achieved.

Main duties of the job

The post holder will be expected to demonstrate strong interpersonal skills, and be able to prioritise and manage own workload using their own initiative to include the following:

Lead the integrated team in the implementation of integrated multidisciplinary care planning ensuring patients receive high quality, patient-focussed nursing care at all times.

Provide day to day leadership, clinical support and supervision of the Neighbourhood team staff within the integrated team as well as supporting the integrated community service lead to provide a seamless service right across the patient pathway.

Demonstrate an understanding of appropriate legislation, policy and working practices e.g. eligibility criteria, which in conjunction with clinical skills may determine the most appropriate course of action, whilst taking in to account the patients / carers preferences and expectations.

Lead, co-ordinate and develop the provision of an effective and efficient integrated care service for patients primarily in the older adult population who have multiple co-morbidities within the extended primary care pathway of a designated neighbourhood team.

Act as the senior point of contact for Trust integrated neighbourhood team staff working with locality GP Practices.
