Job summary
A day shift is 7.5hrs between the hours of 07:30 18.30, Monday
to Sunday including bank holidays). Working weekends will attract an
enhancement payment however you will only be expected to work one weekend in a
four week period.This is a
part time post working 30 hours per week. Minimum of 22.5 hours.
The Worcester City Neighbourhood Team is a friendly, supportive
and hard-working team. We are looking
for an experienced occupational therapist who is keen to deliver high quality
care in the community setting. Worcester
city has two neighbourhood teams Riverside which is based in St Johns House
surgery and Cityside which is based in Elbury Moor surgery. We also have a base at Warndon clinic which
is home to the admin support, shift leads and the therapy team.
The post holder will be integrated within the Riverside and
therapy team. You will be heavily
involved and instrumental in the journey of patients on the intensive support
pathway. This will involve assessing
patients who are being discharged from hospital as well as patients who require
support to prevent admission, identifying the appropriate intervention such as
short-term care support while the patient rehabilitates as well as any
immediate equipment needs. The role is
fast paced and responsive to urgent referrals and works closely with other
nursing and therapy roles. Making a patients home safer and guiding the
promotion of independence is where the OT really adds value to the patient
experience.
Main duties of the job
Your typical day will include coordinating your caseload, triaging
referrals, allocating workload, supporting junior staff and assistants, and
prioritising urgent response assessments alongside colleagues.
Alongside the intensive support pathway, you will be triaging and
assessing patients who require functional occupational therapy assessments in
their own home. This involves manual
handling assessments and the provision of both specialised and standard
equipment needs.

in-service training, regular appraisals, a personal development plan and
clinical supervision.

relatives and other agencies. This role will include organising individual
client case reviews, communicating plans with the MDT, and being responsible for
the ongoing actions in relation to the patient pathway.
Maintaining
contemporaneous clinical notes to the high professional standards of the
College of Occupational Therapy and in accordance with Trust guidelines.
Line
management of band 5 and band 4 team members including training, supervisions
and appraisals.
