Community Specialist Nurse - Palliative Care

Job summary

We are excited to recruiting to our Community Specialist Palliative Care Nursing Team for North Worcestershire.

This is a fixed term post to provide cover for absence for specific purpose that we anticipate will be required for 13 months. We reserve the right to end this contract sooner with appropriate notice.

We are looking for someone to work for 30 hours per week, with the non working day negotiable.

During your time with the team we will work at developing your existing skills in end-of-life care to a more advanced level and provide an opportunity for you gain an insight into the role of the specialist nurses in this field.

You must have read and understood the terms of the Trusts secondment policy and gained permission from your Line Manager to undertake the secondment before applying.

Before applying for this secondment please gain permission from your Substantive Manager to undertake the secondment.

Main duties of the job

The successful candidate will be working collaboratively with Palliative Care services across North Worcestershire including Palliative Medicine Consultants, Palliative Care in-patient Unit, Neighbourhood Teams, specialist palliative care services and specialist services within the neighbouring acute Trusts, Hospices and primary care colleagues to provide advice for patients with a life limiting diagnosis.

The role will involve you supporting the Clinical Nurse Specialists within the service, by undertaking independent reviews of patient symptoms based on their management plan and feeding back to the CNS, as well as liaising with our stakeholders to help with the implementation of these plans and their ongoing delivery.

You will attend and contribute to the weekly MDT meeting and actively support the GSF meetings held by primary care colleagues. As part of this secondment opportunity you will be supported to undertake an audit of your choice around end-of-life care.
