Malvern
Neighbourhood Team is a friendly, flexible, and hardworking team, who are

Staff Nurse, in a beautiful location set at the foot of the Malvern Hills.
We are looking for a passionate nurse who is
keen to help shape a new way of delivering community nursing care. We are a multidisciplinary team of creative
health care professionals who are focused on delivering compassionate,
responsive and patient centred care to meet the needs of our patients.

Main duties of the job
The role will require:

Responsibility for the initial assessment and
review of patients, across a broad spectrum of clinical conditions, considering

person-centred care plan.
Provide skilled clinical care, treatment, advice,
and interventions, including appropriate transcribing/prescribing of medication.
Undertake
continuing health care fast track applications and checklists of on-going
nursing need and where eligible commission packages of care from external
agencies.
Be involved
in triage, allocation of work and leadership of other staff.
