Job summary
We have an exciting opportunity for a Qualified Nurse to join our well-established Memory Services which is a countywide service. The team accepts referrals for individuals of any age with a suspected cognitive impairment or dementia. We provide specialist assessment for people who are presenting with early signs of dementia illness. Following assessment diagnosis feedback is provided, medication monitoring and interventions to support and treat dementia. Working closely with and supporting the dementia advisors service.
This is a permanent post working 30 hours per week.
The service operation between the hours of Monday to Friday 9 till 5 excluding bank holidays. Flexible working patterns will be considered. The team is based in the Benet building at Belmont which is in the grounds of the Belmont Abbey which is peaceful rural location, while being 2 miles from the main city of Hereford. There is plenty of space provided for free parking. The team works between a spilt of office based work and working from home in line with the service requirements and personal preferences.

Main duties of the job
The duties will include:
Working with patients to identify therapeutic goals as part of the overall recovery-focused care plan, using specialist mental health skills.
Adhere to Trust and professional guidelines around the safe administration and storage of the medication.
Effective and timely discharge planning and coordination.
Ensure timely reviews of care plans and assessments.
Undertake and understand physical health observations and take appropriate action where necessary.
Manage a caseload of patients with complex mental health needs (taking on the role of care coordinator where relevant).
Plan and implement patient-centred individual and/or group interventions to achieve therapeutic goals.
Monitor, evaluate, and modify treatment in order to measure progress and ensure the effectiveness of an intervention.
Apply a high level of understanding of the effect of mental health needs and provide training and advice on management of the condition, healthy lifestyles and well-being.
Provide education, consultation and advice to other care settings
Carry out complex risk assessment.
Contribute to the Trusts and Teams clinical governance arrangements and quality agenda, including the setting and monitoring of practice standards.
Participate in undertaking professional cover/duty.
