Senior Community Nurse

Job summary

Part time post, 18 hours per week working between 17:30-23:30 over 7-day rota.

This role is a band 6 Senior Community Nurse within the Evening & Night team, working 17:30-23:30 over 3 evenings a week. The team is countywide, supporting the South of the County, with the flexibility to work countywide as required. You will be part of the triage team from the central hub at Droitwich medical centre for some of your shifts, which involves taking calls from health professionals and direct calls from patients/relatives, overseeing the electronic referrals that come via EMIS and the west midlands Ambulance service system CAD. You will also have clinical responsibilities and visit patients in their own home and support them in remaining at home, preventing admission to hospital and supporting early discharge from hospital.

The post is all worked as unsocial hours hence picking up pay enhancements for all hours worked which makes this post more attractive (more money for less hours).

Main duties of the job

As a community Nurse working with the Evening Team, most of the work allocated to you will of an unplanned nature. The service works as an on-call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work alone.

You will primarily work alone, and have work allocated via a handheld tablet by the triage nurse. You will be able to start your visits from home if the geographics dictates. As a band 6 you will rotate into the triage role based at Droitwich at least once a week. The triage nurse takes the lead for the shift and allocates the work to the team. The county is covered as 2 parts with a triage nurse for the North and South both based in Droitwich.

The patients seen during the evening and night vary immensely and no 2 shifts being the same. We see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention. As a team we often undertake initial assessments of patient in the community.

As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to urgent Care response. These can be patients that have fallen, or requiring further assessment to aid prevention in admission to hospital.
