Community Public Health Nurses (HV). We have 2 posts available, a 37.5 hour
post in Wychavon, based in Evesham and a 22.5 hour post in Wyre Forest, Based
in Kidderminster.
We are looking for keen and
enthusiastic SCPHNs to join our teams. You will lead a skill mix team to deliver
the Healthy Child Programme, driving forward the public health agenda through
collaborative and innovative working.
You will be joining friendly and
supportive 0-19 Public Health Nursing teams. We are looking for SCPHNs who are
keen to use their specialist qualification to manage busy caseloads and support
their skill mix teams. The teams cover both rural and urban areas and being
able to meet the travel requirements of the role is essential. Travel across
the locality, and Worcestershire, will be required.

Main duties of the job
As a SCPHN (HV) you will lead
the delivery of the Healthy Child Programme. You will travel across the
beautiful Worcestershire countryside and through our towns to visit families at
home, in clinic and at locations to meet their needs. Leading a skill mix team,
you will work collaboratively to deliver high-quality, evidence-based
interventions. Your knowledge of local and national issues that impact on the
0-5 population will support your practice. You will have the opportunity to
champion public health priorities, promoting health equity and early
intervention through High Impact Area groups and MECSH. Through monitoring outcomes using evidence-based
tools you will strive to improve service delivery through innovative practice.
Multi-agency working is key to this role, and you will need to be confident
working with other professionals to empower the most vulnerable families to
make positive change. You will be confident in your decision making and report
writing.
