Community Support Worker - SHAPE

Job summary

We are looking for two Community Support Workers to join SHAPE (Supporting Health and Promoting Exercise). The primary purpose of this role is to support the delivery of a health and exercise programme for patients on the Severe Mental Illness Register.

SHAPE is provided by Herefordshire and Worcestershire Health and Care NHS Trust to reduce health inequalities and improve health outcomes by offering patients weekly sessions with advice on health and wellbeing as well as the opportunity to try activities including Zumba, Pilates and Pickleball.

This is a full time post working Monday to Friday. The post holders will be based at Studdert Kennedy House and will support the delivery of SHAPE in several locations. We encourage applications from people with a background in health and exercise, subject to the necessary qualifications.

Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meet Home Office salary threshold you must have the significant experience to be offered top of the pay band AND work a minimum of 35.5 hours per week.

Main duties of the job

The role will require:

Specific knowledge and interest in improving physical health outcomes alongside mental health

Resilience when working with patients who are harder to reach and engage with

The ability to communicate effectively with others at all levels

The ability to problem solve and manage time effectively

Supporting the delivery of information to a large group

Supporting patients in accessing their annual physical health check

A willingness to engage in regular physical activity

Working flexibly to provide the service delivery need across the counties (the service operates from 9 am - 5 pm, Monday to Friday excluding bank holidays)

Working with external stakeholders and signposting patients to onward and supportive services particularly in the VCSE sector

  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
