Job summary
Occupational Therapy clinical leadership is being strengthened within our Neighbourhood Team and we are seeking an enthusiastic and innovative practitioner who will lead to inspire and develop excellent Occupational Therapy practice in the locality. The postholder will work very closely with all clinicians from the community Neighbourhood Teams and work collaboratively on leadership activities and promoting the delivery of safe care. The role will provide clinical leadership including supervision across Worcester and Droitwich Neighbourhood Teams and you will support with clinical visits and triage alongside your therapy colleagues . The teams have an established physiotherapy lead currently undertaking a similar efficient role who you will work closely together with the aim of developing therapy services across the 2 neighbourhood teams . This is an exciting opportunity for an experienced therapist to work in the community and progress with their career in a supportive environment .
This is a 22.5 hour post across the 2 team locations the shifts are Monday to Friday and the core hours are 8.30 to 4.30pm.

Main duties of the job
The post holder will be expected to demonstrate strong interpersonal skills, and be able to prioritise and manage own workload using their own initiative to include the following:
Clinically lead, clinically coordinate and develop the provision of an effective and efficient integrated care service for patients primarily in the older adult population who have multiple co-morbidities within the extended primary care pathway of a designated locality.
Provide day to day clinical leadership, clinical support and clinical supervision of the Neighbourhood Team Occupational Therapy (OT) staff within the integrated team as well as supporting the Integrated Neighbourhood Team Leader and Integrated Community Service Lead to provide a seamless service right across the patient pathway.
Support the effective triaging of patients referred to the Integrated Neighbourhood Team on a daily basis, supporting the coordination of care delivery across the patient pathway.
Provide expert clinical holistic assessment and evidence based pro-active and reactive health care interventions to support patients and their carers to achieve maximum health, wellbeing and independence.
