Community Mental Health Nurse

Job summary

Are you a Band 5 registered mental health nurse with a passion and commitment to providing excellent, compassionate, trauma informed care as part of a supportive multi-disciplinary team?

The Assertive Outreach Team is a community based mental health team comprised of psychiatrists, psychologists, qualified nurses, OTs, nursing associates and support workers working from a staff base in the centre of Hereford.

We provide assessment and treatment for patients who experience severe and enduring mental health conditions who have historically struggled to engage in mental health treatment and who may have dual diagnosis with co-morbid substance misuse. We are recovery focused and work flexibly with our patients to meet their complex and changing needs.

You will be required to work 37.5 hours per week over a 9-5 Monday to Friday service. Flexible working is available. You will work with a supportive and valued team of professionals, receive regular supervision and have CPD and career progression opportunities.

As a Band 5 nurse you will be supported to develop your skills in a safe and nurturing environment. You will be supported through your preceptorship, if applicable, and be given many opportunities to experience all aspects of working in the community.

Main duties of the job

  • Undertaking comprehensive risk assessment
  • Developing care plans which are collaborative and patient centred
  • Support you patients to identify early warning signs of relapse.
  • Liaise with other agencies such as housing, substance use services, probation, police and social security.
  • Arrange regular reviews with team psychiatrist
  • Update the team weekly on the progress of your patients.
  • Implement interventions as directed by the Multi-disciplinary team
  • Liaise with carers and relatives providing support and signposting
  • Provide information to your patients on strategies to improve healthy lifestyle.
  • Administer medication and monitor for side effects
  • Keep up to date with all statutory and mandatory training.
  • Undertake training specific to your role
  • Attend an annual appraisal
  • Attend monthly clinical supervision and reflective practice sessions
  • Contribute to all multi-disciplinary team meetings and medical revies.
  • Support ongoing service development initiatives
