This is an exciting opportunity to be part of the delivery of a new service and to play a significant part in shaping and developing this service. There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders in particular with Acute Trust colleagues.
The service will be countywide across Worcestershire so the ability travel across the county will be expected. Main duties of the job
There will be the requirement to support the assessment and management of acutely unwell patients in the Community who otherwise might have been in an Acute Hospital setting with the support of a dedicated medical model for the Hospital @ Home.

You will have the ability to prioritise and triage the needs of patients, making any referrals for further investigations where appropriate in line with local and practice guidelines, whilst ensuring your clinical practice is safe and effective and escalating to clinical leadership as required.
