Job summary
We have an exciting opportunity for Band 3 Healthcare Assistants who have a passion for admission prevention to join a core team to deliver a new and developing countywide Hospital at Home (H@H) service in Worcestershire.This is full time post, but we would consider applicants looking for part time.
The post is based in the Single Point of Access (SPA) Worcester.
This is an exciting opportunity to be part of the delivery of a new service and to play a significant part in shaping and developing this service.
There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders in particular with Acute Trust colleagues.
The service will be countywide across Worcestershire so the ability travel across the county will be expected. The service will be operational 8am-8pm seven days a week including Bank Holidays so the post holder will be expected to work shift patterns to enable cover of the operational hours.
Please note the Trust will not routinely provide sponsorship to work for this role. In addition to meet Home Office salary threshold you must have the significant experience to be offered top of the pay band AND work a minimum of 35.5 hours per week.
Main duties of the job
There will be the requirement to follow treatment plans to support the assessment and management of acutely unwell patients in the Community who otherwise might have been in an Acute Hospital setting with the support of a dedicated medical model for the Hospital @ Home.
The ability to work independently is essential as the post holders, who will need to follow the treatment plans for patients whilst ensuring your clinical practice is safe and effective and escalating concerns/deterioration to clinical leadership as required.
