Job summary
This is a part time post working 30 hours per week.
We have an exciting opportunity for a Qualified Nurse to join our well-established Care Home In Reach Nurse, which is a countywide service. The team provides support to residents of care homes living with dementia or those over 65 with a functional mental health illness. The service is open Monday to Friday, 9-5, excluding bank holidays. Flexible working patterns will be considered. The team is based in the grounds of the Belmont Abbey, which is a peaceful rural location, 2 miles from the main city of Hereford. There is plenty of free parking. We pride ourselves in providing outstanding care to patients. Main duties of the job
The main duties of the role will include-
Manage a caseload of patients to ensure they receive a timely assessment, diagnosis and treatment
Monitor treatments
Effective and timely discharge planning and coordination.
Undertake and understand physical health observations and take appropriate action where necessary.
Monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention.
Apply a high level of understanding of the effect of dementia and provide advice on management of the condition, healthy lifestyles and well-being.
Carry out complex risk assessment.
Timely and accurate reporting and escalation of incidents and safeguarding concerns.
Attend weekly multidisciplinary meetings.
Participate in undertaking professional cover/duty.
