Occupational Therapist - Integrated Community Stroke Service

Job summary

If you are a Band 6 OT looking for a part-time job within the Worcestershire Stroke Pathway, we have a post available within our Integrated Community Stroke Service which covers the county of Worcestershire. We are passionate about improving the quality of life for Stroke Survivors.

We would love to hear from Band 6 OTs with experience in stroke services that would like to work within both inpatients rehabilitation and community stroke on a rotational basis.

As a trust we offer a robust induction period and continue to promote opportunities for development through 1:1 clinical supervision sessions, peer working and in-service training. We have a well-established stroke pathway and perceive our acute, inpatient and community stroke colleagues as one team, all striving for the best quality of life for our stroke survivors and their families.

This is a part time post working 22.5 hours per week.

Main duties of the job

Day to day, you will be undertaking comprehensive assessments of mild-moderate-severe stroke patients including those with multiple pathologies/co-morbidities, using specialist OT assessment tools to identify their personalised goals alongside the MDT. You will formulate individual programmes of rehabilitation using clinical reasoning and utilising a range of treatment skills, targeting realistic and measurable, patient-led goals. It is also expected that you will offer support and education to carers/families.

You will attend the weekly MDT in which the team will discuss the progress of patients and identify goals that are achieved and ongoing. As a Band 6 in the community team you will also participate in referral management which will include the triaging of referrals, liaising with other health care professionals within the Trust/Acute Trust and surrounding borders and third-party organisations.
