a community environment, this is a fantastic opportunity to join the Evesham Neighbourhood Team. We
are looking to recruit an enthusiastic and committed band 6 Physiotherapist to
be based at Evesham Community Hospital. Our team covers 7 GP surgeries situated
in the beautiful countryside of Worcestershire.

Occupational Therapists, Health Care Assistants, Advanced Clinical
Practitioners, Nursing Associates and Administrators who all work together alongside
our GP partners as part of our Multidisciplinary Team approach to support
people in their home environment

The role will require:

Act as line manager to junior staff and assistants
including undertaking the annual staff appraisal and development reviews for
these staff.
Work as an autonomous practitioner within an
integrated team providing timely and expert professional assessment and
treatment interventions to patients either in their own homes or other
community settings. This will be achieved through a combination of hands-on
expert clinical skills and knowledge as a Physiotherapist to treat, educate and
support patients and by working with patients and their families to assess,
plan and implement appropriate care plans and treatment.
Provide case management for a designated caseload of
patients who have immediate and / or complex care needs.
Undertake
the comprehensive assessment and diagnosis of patients including those with a
highly complex presentation, using investigative and analytical skills.
Formulate individualised management and treatment plans using clinical
reasoning.
