Service Review Lead - Community Mental Health

Job summary

This is an exciting opportunity to lead and inform the review of our community mental health services working alongside the Associate Director and their Deputy within the Primary Care and Community Mental Health SDU.

In June 2024 the NHS Operational Planning Guidance required all systems to Review their community services by Q2 2024/25 to ensure that they have clear policies and practice in place for patients with serious mental illness, who require intensive community treatment and follow-up but where engagement is a challenge. HWHCT and HWICB undertook a system review with staff and partners in August 2024 and submitted the Maturity Index tool provided to NHSE. From our local review several areas of work were identified across our health and care system and the successful postholder will be pivotal in driving this work forward.

The post holder will be full time and have project management support working alongside them. They will report directly to the Deputy Associate Director and will form part of the wider SDU Senior Management Team. They may be required to travel and work across multiple sites. The work base is yet to be determined and can be discussed in more detail at interview.

Main duties of the job

This is a management role requiring the post holder to:

Lead the community-based recovery-orientated mental health team/service ensuring that all resources, including staff, are used, and monitored effectively to deliver a recovery-focused service while remaining within an agreed budget.

Review current pathways to ensure they are fit for purpose and meet the needs of the local population including forensic pathway, criminal justice, housing, substance use.

Actively work with wider systems partners and commissioners where indicated to review, remodel, enhance or establish required pathways.

Actively participate in planning and developing the service to provide patients with a person-centred approach which is relevant to their needs and is recovery focused.
